Sankofa Counseling staff are available at various Portland Public Middle and High Schools for the 2024-25 school year. If you are a BIPOC student or parent seeking support, follow the steps below to connect with school-based counseling services.
Step 1A: Submit a Release of Information (ROI) between Sankofa and PPS using the link below.

Complete the Online Referral Form: Click your school's link above to access and fill out the referral form.

Student and Counselor Meeting: The student will meet with a counselor for an introduction.
Initial Screening: During this meeting, the counselor will assess the student’s care needs and discuss scheduling options.

Complete Intake Packet & Informed Consent: Students and parents will need to fill out the necessary forms to begin services.
Schedule Appointments: Set up the initial appointment and plan for ongoing sessions.
