School Based Counseling Services for Portland Public Schools
Sankofa Counseling providers are available at various Portland Public Middle and High Schools for the 2024-25 school year. If you are a BIPOC student or parent seeking support, follow the steps below to connect with school-based counseling services.
Step 1A: Confirm student is a fit for services using this form.
Step 1B: Submit a Release of Information (ROI) between Sankofa and PPS using the link below.
Step 1C: Find and submit your school’s referral form using the links below.

A) Confirm program eligibility using the pre-referral checklist.
C) Complete the Online Referral Form: Click your school's link above to access and fill out the referral form.

A) Once you've submitted your referral, the Sankofa Counselor will reach out to student to schedule an initial meeting.
B) The student will meet with a Sankofa Counselor for an initial meet/greet or initial appointment. This meeting is an opportunity for the student to share about themselves, and for the counselor to understand their unique needs and goals.

A) Sessions Begin: Set up the initial appointment and plan for ongoing sessions.
B) Complete Intake Packet & Informed Consent: Students and parents will need to fill out the necessary forms (these need to be completed by the 3rd session to continue services).
